What is Supplemental Medicare Insurance and How Does it Work?

Medicare Supplement Insurance, also known as Medigap, is a type of health insurance that covers some of the health care costs that traditional Medicare coverage doesn't pay for. These expenses can include copayments for doctor's office visits, coinsurance, and plan deductibles.

How Does Medicare Insurance Work?

Medicare Part A is the most common Medicare insurance plan because every U.S. resident receives it from the government (at no charge) when they reach age 65. Part A Medicare pays for 80% of your inpatient medical expenses including emergency hospital bills.

Part B is the second-most popular type of Medicare coverage. Unlike Part A, Medicare Part B is not provided as a free retirement benefit, it must be purchased by the insured. Part B Medicare pays for 80% of your outpatient expenses including routine visits to your primary care doctor.

Supplemental Medicare Insurance pays the remaining 20% of the medical bills that Medicare Part A and Part B don’t cover. Some Medical Supplement Insurance plans also pay your coinsurance costs, deductibles, copayments, and even prescription drug costs.

With traditional Medicare coverage and Supplemental Medical Insurance in place, you can choose your doctor just like a PPO plan. If you need to see a specialist, no referrals are needed from your primary care doctor, and your coverage works at every hospital and doctor's office that accepts Medicare.

The downside to Medicare Supplement insurance is that it can be more expensive than others plans that limit your care to one provider. It’s also important to note that Medicare offers limited benefits for vision and hearing and some expenses like hearing aids are not covered.

Do I Really Need Medicare Supplement Insurance?

Not everyone needs to supplement their Medicare coverage. If you’re in excellent health, take no medications, and rarely see the doctor, you might money without a supplemental plan. However, for most people, especially those living on a fixed income, it’s not worth the gamble.

Traditional Medicare only covers 80% of the cost of most health-related expenses, so without a supplemental plan, and you are still responsible for 20% or more of the medical bills you incur. This includes routine visits to your primary care doctor and unexpected trips to the hospital.

The average stay at a hospital costs about $16,000, not including surgery. If your medical condition requires surgery, you can expect to owe thousands of dollars on top of any operating room costs, surgeon’s fees, anesthesia expenses, and pre-surgery treatments.

Here’s a breakdown of the most common surgeries and their cost in 2018:

  • heart valve replacement: $170,000
  • heart bypass: $123,000
  • spinal fusion: $110,000
  • hip replacement: $40,364
  • knee replacement: $35,000
  • angioplasty: $28,2000
  • hip resurfacing: $28,000
  • gastric bypass: $25,000
  • cornea: $17,500
  • gastric sleeve: $16,000

Paying for a surgery like one of these could spell financial disaster for someone that doesn’t have adequate health insurance. In the next section we’ve provided some actual rates for supplemental Medicare insurance plans by state.
